HPA (ImmForm) Pneumococcal Uptake Library

The purpose of this data collection is to provide the HPA via the DH ImmForm Website with data on vaccination uptake in adults aged 65 and over as per the Chief Medical Officer's (CMO) letter.

This library only includes data collection for those receiving the Pneumococcal Polysaccharide vaccine (PPV) which is recommended for older people and at risk groups over the age of two.

PRIMIS are hosting the upload for this campaign. Practices can upload data at the click of a button by loading the anonymised summary sheet in CHART and selecting the upload data to web icon.
