Information Services

The strength of PRIMIS in improving primary care data quality and information management lies in its integrated approach to education and training linked to data extraction, analysis and feedback. The PRIMIS approach is not only to educate end users on the reasons and methods for creating and maintaining accurate clinical records, but to provide them with the tools to record, extract, analyse and use their data to improve patient care.

The following tools are available to members of the PRIMIS Hub and are used by PRIMIS in providing information services to a number of NHS and commercial clients:

CHART

CHART (Care and Health Analysis in Real Time) is a software tool from PRIMIS that helps GPs improve patient care by analysing the data held on their clinical computer systems. CHART works in conjunction with CHART Online

CHART Online

The CHART tool permits easy and secure uploading of summary data to CHART Online, which then allows GP practices and PCT staff to compare the data against national and local peer sets.

CHART Query Libraries

The query sets available to you will depend on your level of membership of the PRIMIS Hub. Take a look at the full list of query libraries and the membership options to choose the level of membership that suits you.

GRASP-AF (Guidance on risk Assessment & Stroke Prevention for Atrial Fibrillation) is a tool developed initially by the West Yorkshire Cardiovascular Network, the Leeds Arrhythmia team and PRIMIS. NHS Improvement is supporting the national roll out of the tool and promoting its use as part of a systematic approach to the identification, diagnosis and optimal management of patients with AF to reduce their risk of stroke.

GRASP AF has two components: a CHART query library and CHART Online toolkit. Practices in England can use both free of charge by registering for free basic membership of the PRIMIS Hub.

Instructions for CHART, CHART Online, CHART libraries and MIQUEST are available to full members of the PRIMIS Hub in the Training Centre.
